SPORTS PHYSICALS
All Football Players Need their Physicals Completed before July 6th., COC
FastMed is offering Student-Athletes the opportunity to get their Sports Physicals completed at discounted price. The coupon is available in email addressed to everyone, called Sports Physicals - Discount. All Physicals are required to be completed on AIA Clearance Packet forms, which are available at this link https://www.cusd80.com/Domain/5338, near the bottom of the web page. After the forms are completed they will need to be scanned and uploaded onto the "Register My Athlete" website. The website link for "Register My Athlete" is available on this website, on our Links tab on the Home Page.
